Immunization Requirements
The Commonwealth of Virginia requires certain immunizations prior to the enrollment of a student in a public school. These immunization requirements include:
DPT (Diphtheria, pertussis [whooping cough], tetanus) - A minimum of 3 with 1 after the 4th birthday. After age 7 yrs, the pertussis is not routinely administered. Additional vaccination against pertussis is available for teens, but is not required at this time.
Tdap Booster - All students who will be in sixth grade after July 1, 2006, are required to have a tetanus, pertussis, and diphtheria booster, prior to the first day of school. If a tetanus containing vaccine has been administered in the past five years, this booster is not required. Documentation of this vaccine may appear as DPT, T, Td, Dtap, and/or Tdap.
Polio - A minimum of 3 with 1 after the 4th birthday.
MMR (measles, mumps, rubella) - must be received on or after 12 months of age.
Second measles (may be in the form of a second MMR) - required for all school children.
Hepatitis B – A series of 3 shots for children. Immunization series must be started before the child starts class. Adult dose is also acceptable for older students and consists of only 2 shots.
Varicella (chicken pox) - Required for all children born on or after January 1, 1997. If the child has had the disease, documentation from a physician is accepted in lieu of vaccination
Exemptions include written documentation from a physician stating the child should not be immunized for medical reasons and include the specific medical reason.
Religious exemptions must be signed, witnessed, and notarized.
